EFFECT: Changes the circumstances under which parents are not required to obtain a signed statement from a health care practitioner on exemption forms by requiring that parents demonstrate membership in a religious body or church in which the religious beliefs or teachings of the church preclude a health care practitioner from providing vaccination to the child, instead of medical treatment to the child.
