On page 4, after line 24 of the striking amendment, insert the following:

"(c) The school must provide to all parents and guardians of enrolled students a form that the parent or guardian can use to make the written requests described under (a) of this subsection (7)."

EFFECT: Adds that a school must provide to all parents and guardians of enrolled students a form that the parent or guardian can use to make a written request to either excuse his or her child from comprehensive sexual health education or to review the provided comprehensive sexual health education curriculum.
