On page 4, line 13, after "designee." insert "The school or applicable school district must provide to all parents and guardians of enrolled students a form that the parent or guardian can use to make the written requests described in this subsection (7)."

EFFECT: Adds that a school or applicable school district must provide to all parents and guardians of enrolled students a form that the parent or guardian can use to make a written request to either excuse his or her child from comprehensive sexual health education or to review the provided comprehensive sexual health education curriculum.
