We provide a place on your quarterly premium reports to let us know about any changes you have made. If you need help determining if changes to your account are necessary, you can call your account manager. Depending on the type of change you are making, you may need to complete a new business license application. The name and phone number of your account manager can be found on your quarterly premium report or annual rate notice.
