What components contained in the service level assessment module determine a service level and/or number of hours?

The service level assessment module contains two components that are used to determine a service level and/or number of hours for the following:
(1) The CARE assessment for medicaid or waiver personal care services, as defined in chapter 388-106 WAC; and
(2) The DDD seizure acuity scale as defined in WAC 388-828-7040 through 388-828-7080.
