Document Title: Provider Notice #14-05.

Subject: Home Infusion Therapy/Parenteral Nutrition Program Medicaid Provider Guide.

Retroactive to dates of service on and after January 1, 2014, Washington apple health (medicaid), administered by the health care authority (HCA) has updated the Home Infusion Therapy/Parenteral Nutrition Program Medicaid Provider Guide to:

•	Add information about how to verify a patient's eligibility in ProviderOne.
•	Remove HCPCS code A4245 described as "Alcohol wipes, per box." HCPCS code A4245 can be found in the agency's current Nondurable Medical Supplies and Equipment (MSE) Medicaid Provider Guide.
