Document Title: Numbered Memorandum 05-18 MAA. Subject: Prescription drug program: Prior authorization

Effective Date: May 2, 2005.
Document Description: Effective the week of May 2, 2005,
and after, the Medical Assistance Administration (MAA) will
implement the following changes to MAA's prescription drug
program:
