Document Description: Effective for dates of service on and after July 1, 2010, DSHS will implement the updated medicare physician fee schedule data base year 2010 relative value units; the updated year 2010 relative value guide base anesthesia units; the updated medicare clinical laboratory fee schedule; the updated medicare average sales price drug files; consultation services policy (99241-99245 and 99251-99357); changes to vision care services; and the technical changes listed in this numbered memo.
