This memo serves as notice that the pharmacy quality assurance commission (commission) is rescinding Policy Statement P006, Verification of Age by Applicant, which was filed February 1, 2021, and published in WSR 21-04-119.

The policy statement authorized the department of health (department) to accept signed applicant's attestation on the official application, as required under WAC 246-945-150, as proof that an applicant meets the regulatory age requirement under RCW 18.64.080 of 18 years of age or older. The commission is rescinding this policy because the commission has determined that by completing the application, the applicant is attesting that the information provided is true and correct.
